What to Expect
How to Prepare for Your Visit
To make the most of your appointment, please bring:
- A list of your current medications, including dosages.
- Any relevant medical records, including lab results or imaging reports.
- Your insurance card and a valid photo ID.
- Questions or concerns you’d like to discuss with our providers.
Arriving 15 minutes early is recommended to ensure you have enough time to complete any additional forms.

At this time we only take appointments by phone or with a doctor's referral. If you would like to schedule a visit, please give us a call at 404-355-1799.
